The app will allow you to record observations for injuries that have already been logged for a service user.

It is not possible to report new injuries using the app at this time. In order for an injury observation to be recorded, it must first be logged on their record by the office.

Which information is recorded?

The following information will be asked for when recording blood glucose observations.

Status - was the reading successfully taken?

Here you can record how successful the observation. For example "Completed" or "Refused".

Time - when did it happen?

Add the time that the reading was taken.

By default this will be set to the current time, but it can be adjusted in the event that you are completing the records after the visit has finished.

The time recorded must be within the visit time, you will not be able to add a time that is before the start of the visit or after the end of the visit.

Current status/severity

This is the current state of the injury, and how severe it is. You'll be presented the following options:

  • No action necessary

  • No harm

  • Needs monitoring

  • Severe symptoms

Progression - is it better/worse than the last observation?

How do you think the injury looks compared to the last time it was observed? Is it on the mend, or is it getting worse? You should choose from the following options:

  • None

  • Green

  • Amber

  • Red

Reporting a concern

You may wish to report a concern if you've noticed something out of the ordinary; for example:

Fred’s leg wound is weeping. Fred's daughter has contacted the district nursing hub.
