Accessing the Care Circle

This article is for family members that have been invited to access the Family Portal for a service user.

⚠️ Care agencies have control over the level of detail shown within the Care Circle. The detail shown on this page is a guideline only.

Receiving the invitation

Once you’ve received your Family Portal Invitation via email, you will need to set up a password for your account. Click the Create your account button to do this. Invitations are only valid for a week; if yours has expired you will need to request a new invitation from the care provider.

This will take you to the registration page (pictured below) which has auto populated your email address, along with a unique invitation token which grants you access to the Family Portal. All you need to do is choose a memorable password and then click Create Your Account.

Once you’ve chosen your password and your account has been set up, you’ll be taken straight to the Family Portal

Accessing the Care Circle

At this stage, you may wish to bookmark the page for easy access later, and make a note of the provider’s unique URL in the address bar (highlighted in the screenshot below, please note this will not be the same for you).

If you are unsure, please contact the care provider for clarification. For data security, we are unable to provide you with this information.

The link to the Family Portal that was sent to you in the email invitation will expire, so you will need to keep a record of the URL to access and log on to the web environment in future.

Upcoming & past visits

The main page of the family portal shows upcoming & past visits. Each visit will show a summary, including the type of Visit (Personal Care for example), time, carers, and how many notes/tasks were logged.

Calls that have been completed will be indicated with a green tick. Click the visit to see more information about it.

Visit Details

Here you will be able to see the following:

  • The attending carer(s) & type of visit

  • Tasks, medication, and any comments that have been left by the carer

  • Any notes left by the carer

To go back to the main screen, click on the arrow above visit details.

Viewing a client

You can access individual records for the person(s) you are able to view the care circle for (you may have access to multiple people if needed. Select the menu icon in the top left before clicking into client's record. There are a few tabs across the top:

  • Overview - A snapshot of medical conditions, the latest visit and any documents made available

  • Profile - The client's personal details, contact information and important contacts

  • Medical - Medical conditions, injuries and medication

  • Visits - Past and upcoming visits, including completions, notes and task completions

  • Observations - Observations recorded in the past 14 days within visits

Changing your password

If you need to change your password, click the ⋮ icon at the top right of the main screen, then Change password and follow the instructions.
